Tell her to go to a program. She can't get clean unless she wants to

Addiction isn't something that anyone else can really help with, beyond like giving them info on where to go for help and things like emotional support. The addict needs to deal with it on their own. Also she really needs rehab. Try finding a free place in your area, if you can't find anything then look a little further away. It might take a while but they are out there. Try go ogle, also check with the Salvation Army and local health and mental health departments.
Your grandma should not let her move back in until she's been through rehab and is clean and going to meetings.

talk to a community Social Worker, or visit a mental health clinic, they should be able to provide the information that you need.
When a person is addicted to anything, there is very little anyone can do to intervene, the individual themselves have to take the steps, the most you can do is locate the steps for her to take.
and one other very important thing to know about addiction and addicts, is that the substance they are addicted to is their only Best Friend, and they usually won't let anyone or anything come between them.
and they will say anything, in order to get close to a source of finance funds or property to pawn in order to get their fix. Their new friend takes priority over everyone that is or was in their life.
An Addict started with a Brain Chemistry Imbalance that Kept their mind in constant torment, and they start to self medicate to get some degree of relief, however their choice of substance is usually addictive, which brings a whole new Chaos into their life.
If they don't get straight, they usually lose Family - Friends - Home - Car - Job, and unfortunately, some have to hit absolute bottom , before they are forced to get help or Die.
There are Some things in Life that We have absolutely No power to Change, and We have to Accept that, to maintain your own balanced brain chemistry.
Try to do the Best You can, Your Mother is Your Mother For the rest of Your Life, -That Never Changes.
You might consider Forgiving Her for not having the Strength and resolve to resist the urge,
Best of Luck!
